Please use this form if you wish to pay by check, money order, or purchase order.

Items marked with an asterisk are required.
If you are a CWA member, please provide your CWA Member Number and Membership Expiration Date.
If you have a Registration Code, please provide it below. You may enter only one code per registration session.
Please provide complete registration information for each attendee after clicking continue.
Members may send paid staff at the member rate if the membership is current at the time of registration.
If your membership is not current and you register as a member, you will be invoiced for membership on terms "due on receipt."
A 10% discount will be applied to the conference registration fee for groups of three or more attendees.
You will receive an automatically generated receipt via e-mail (check your spam folder if it does not appear in your inbox).

Payment and Cancellation Policies
Please note: the conference registration fee covers conference attendance and printed material only. Additional registration fees apply to pre-conference workshops. Registration fees are not transferable to persons outside of your organization or to subsequent events. Cancellations or transfers must be made in writing to the CWA no later than April 26, 2013. If you cancel your registration prior to March 8, 2013 you are entitled to a 75% refund of your total payment, prior to April 26, 2013 a 50% refund of your conference registration fee only (pre-conference fees will be forfeited). No refunds after April 26, 2013 and no refunds for no-shows, no exceptions.
