I hereby authorize Climbing Wall Association, Inc. to initiate debit entries through their billing agency Authorize.net against my credit card and the card provider to debit the same from such account.
This authority is to remain in force and in effect for an initial one year term; thereafter the payment amount as indicated on the form will be debited at the frequency indicated until the CWA has received notification from me of its termination.
Each debit entry is subject to a $2.95 billing fee per transaction. Debit entries that are returned by my credit card company are subject to a return item charge of $25.00. Such returned item charge shall be automatically deducted against my credit card account indicated herein.
