Please use this form if you wish to pay by check, money order, or purchase order. Please provide all required information, items marked with an asterisk are required. Please provide a complete registration form for each attendee. Members may send paid staff at the member rate if the membership is current at the time of registration. If your membership is not current and you register as a member, you will be invoiced for membership on terms "due on receipt." You will receive a receipt and conference information via e-mail.

Payment and Cancellation Policies
Please note: the conference registration fee covers conference attendance and printed material only. Additional registration fees apply to pre-conference workshops. Registration fees are not transferable to persons outside of your organization or to subsequent events. Cancellations or transfers must be made in writing to the CWA no later than April 16, 2010. If you cancel your registration prior to February 19, 2010 you are entitled to a 75% refund of your total payment, prior to April 16, 2010 a 50% refund of your conference registration fee only (pre-conference fees will be forfeited). No refunds after April 16, 2010 and no refunds for no-shows, no exceptions.
